What do I need to bring to register my child?
To register a child for the first time in any Florida school, you will need:
- Birth certificate
- Immunization records on an HRS 680 form for MMR (mumps, measles, and rubella), DPT (diphtheria, pertussis, and tetanus) and OPU (polio)
- Proof of a physical examination within the last year, or one scheduled within 30 days
- Proof of residence, including a homestead exemption card, lease/rental agreement, declaration of domicile for residents new to Florida
Important Immunization Information
There is a state requirement that seventh graders must have a series of three Hepatitis B injections, a tetanus booster and a second measles shot. They must offer proof of these immunizations before they can enroll in the seventh grade.
Where can my child get immunization shots and a physical exam?
See your private physician or visit one of the Orange County Health Department five clinics. Call 407/836-2600 for clinic locations.

When should I register my child for school?
You may go to the school your child will attend at any time to register him or her for school. If you are new to the area, you are encouraged to register your child as soon as possible. Previous school records or report cards help schools place students.
